The property is a 9504+/- sf developed site. The property overlooks Mill Creek (tidal) and is separated from the
creek by Mill Creek Lane, a paved road. A salt marsh lies adjacent to the waters of Mill Creek. The entire site is
also designated as Land Subject to Coastal Storm Flowage (Zone AE, EL 11 and EL 13- NAVD 1988). The 35', 50'
and 100' Buffers are measured from the landward edge of the salt marsh. The Riverfront (approximately 9504 so
extends 200' landward of mean high water.
An existing dwelling, garage and shed are located on the property. The dwelling was constructed in 1960.
Project Description
The work includes the "after the fact" construction of a patio, steps, wall and grading in the buffer zone to a Salt
Marsh and on Land Subject to Coastal Storm Flowage.

— 608sf of a wall, steps and patio were constructed in the 50' to 100' buffer
Performance Standards
The work was confined to the westerly side of Grist Mill Lane. The Salt Marsh was not impacted by the activities.

All filings must be made on Town of Yarmouth forms. The filing deadline is two weeks prior to the next
scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall
result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to
the meeting schedule. Contact the Conservation Office if you need assistance.

Detailed narrative of the project including existing & proposed conditions, construction sequence, type of
equipment, staging locations, drainage and stormwater, erosion controls, invasive species management
and alternative analysis. The narrative shall include how the project meets performance standards per
310 CMR 10.0 & TOY Wetland Regulations, delineation sheets (if BVW or vegetated wetlands are
present) or other resource area calculations, and supporting information.

the Conservation office with proof that this has been done or the filing will not be accepted.
