Chapter 91, the Public Waterfront Act, and the Waterways Regulations at 310 CMR 9.00. Any change in use, alteration of the structure or change in the information and plans that were submitted to MassDEP may void this Certification. You are required to record the enclosed Certification (1-page Issued Certification and 2-page plans) as described below. Also enclosed is a copy of the General License to which you certified compliance. It describes the requirements and conditions that apply to your proposed structure. Do not record this copy of the General License at the Registry of Deeds; this is only for your information and for your records. RECORDING THE GENERAL LICENSE CERTIFICATION This Certification becomes valid, and work described in the Certification may begin once it is recorded within the chain of title of the affected property in the Registry of Deeds or, if registered land, with the Land Registration Office. In the case of recorded land, the Certification shall also be noted in the Registry’s Grantor Index under the name of the owner of the land upon which the project is located. In the case of the registered land, the Certification shall also be noted on the Land Court Certificate of Title of the owner of the land upon which the project is located. Failure to record this Certification as described above within sixty (60) days of the date of issuance will render this Certification void pursuant to 310 CMR 9.18. Failure to notify the Department of the recording of this Certification is a violation of 310 CMR 9.00. Letter height shall be a minimum of ½”. • The MassDEP Chapter 91 Waterways General License Certification Number shall be displayed on all signs. • Each sign shall indicate that public passage is allowed for on-foot lateral access along the shoreline for the purpose of fishing, fowling, navigation and strolling. • Signs shall be posted on both sides of the pile-supported structure, at the mean high water shoreline. Note to Licensee: these specifications do not need to be recorded with the License at the Registry of Deeds An example of an acceptable sign is provided below. GLC #WW24-0000044
