The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Contact the Conservation Office if you need assistance. Submitting an Application: Does the property have any outstanding/expired Orders of Conditions? If so, please file a Request for Certificate of Compliance for each outstanding/expired Order, as the Commission may not issue a new Order until any outstanding/expired Orders have been closed and recorded. (*see page 2) Hardcopy filing dropped off or mailed to the Conservation Office including: 1 Administrative Checklist 1 complete NOI application WPA form 3 with original signatures. Typed signatures will not be accepted. Detailed narrative of the project including existing & proposed conditions, construction sequence, type of equipment, staging locations, drainage and stormwater, erosion controls, invasive species management and alternative analysis. The narrative shall include how the project meets performance standards per 310 CMR 10.0 & TOY Wetland Regulations, delineation sheets (if BVW or vegetated wetlands are present) or other resource area calculations, and supporting information. 1 100’ radius map, current abutters list identifying the property owners who are to be notified per 310 CMR 10.00 and abutter notification form. Abutters list must be certified by the Town Assessor’s office. The Assessor’s office requires 7 days advance notice. All abutters must be notified via certified mail. Certified Mail Receipts (PS Form 3800) for all abutters. Green cards to be submitted at the hearing. 1 original and 7 copies of the plan, folded separately, right side out with title and project address visible. All plans shall reference NAVD1988 unless otherwise noted. Notice of Intent 2 Andrina Road West Yarmouth, MA PROJECT DESCRIPTION 1.0 INTRODUCTION The proposed project at 2 Andrina Road in West Yarmouth, MA (the Site) consists of permitting a raze and replacement of the existing residential home and the installation of a dry well (the Project). This application is being submitted in accordance with the Town of Yarmouth Wetlands Protection Bylaw (the Yarmouth Bylaw) and the Massachusetts Wetlands Protection Act and associated regulations (the WPA). 2.0 SITE DESCRIPTION The property located at 2 Andrina Road in West Yarmouth, is bordered by private residences and a Coastal Bank located to the north of the intersection of Andrina and Lewis Bay Road. The lot is currently developed with one (1) primary dwelling, one (1) garage, concrete walkways, a paved driveway, and existing underground utilities. The Project is not directly within jurisdictional wetland resource areas that are protected under the Yarmouth Bylaw and the WPA. However, the Project is within the 50-100-foot Buffer Zone to Coastal Bank. 3.0 Existing Conditions Existing property features within the 50-100-foot Buffer Zone to Coastal Bank include a portion of the existing residential home, the garage, concrete walkways, and the paved driveway. 4.0 PROPOSED ACTIVITIES The purpose of this application is to authorize the following proposed activities: Proposed Raze and Replace: The proposed project involves the raze and replacement of the existing residential home and the installation of a dry well. The existing house is approximately 2,555 square feet (sf) within the 50–100-foot Buffer Zone to a Coastal Bank. The proposed replacement home will be 2,635 sf, resulting in only an 80 sf increase in area within the Buffer Zone. Importantly, both the existing and proposed homes are located approximately 77 feet from the top of the Coastal Bank with Lewis Bay Road between the Coastal Bank and the project Site, which provides an added physical buffer to the resource area. Additionally, the proposed home expansion is directed away from the Bank. To provide added stormwater mitigation from roof runoff, a 500-gallon drywell surrounded by 1-foot of stone will be installed on the southern side of the property, outside of the 0-100-foot Buffer Zone to Coastal Bank. No fill material is proposed as part of this project (note: all work falls outside Land Subject to Coastal Storm Flowage). The proposed project activities will permanently impact 80 sf 50-100-foot buffer zone to Coastal Bank due to increasing the square footage of the new house. The vast majority of the proposed expansion falls outside of the 100-foot buffer zone. Construction Sequencing: The proposed project activities will follow a typical construction timeline involving the installation of sediment controls at the limit of work, site preparation, establishment of staging areas, pouring of concrete, landscaping restoration, and final site stabilization with loaming and seeding of all disturbed areas. Typical construction equipment will be utilized for this project. Access to the site will be gained from the existing gravel driveway off Lewis Bay Road. 5.0 MITIGATION A line of staked straw wattles and/or silt fences will be installed at the limit of work, closest to the coastal bank prior to the start of project activities. These controls will remain in place and in operational condition until the final site stabilization is complete. All construction materials will be staged outside the resource area and buffer zone, to the extent feasible. All debris from the home improvement activities will be contained within appropriate receptacles and/or immediately removed from the Site. Upon completion of all construction work, disturbed areas will be restored and seeded for stabilization.
