Ojala P.E., P.L.S. Daniel E. Gonsalves, P.E., S.E. Craig J. Ferrari, E.I.T., S.E. In January of 1992, the Commission issued an Order of Conditions for a proposed revetment at the above referenced site. A site visit was made in April of 2024. Photographs representative of the site are attached. The work was found to be in general conformance with the approved plans with some minor deviations. These changes include 4 wooden walkways in lieu of the approved 3, less than 10' buffer at the top of the bank in certain areas, and the size of stones utilized appear to be smaller than specified on the design plan. The wall is starting to fail and break apart with erosion at the top of the bank so exact conformance to the plans is difficult to gauge. A new notice of intent has been filed for repair of the wall. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
