B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
Office (see http://www.mass.gov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office-

Certificate of Compliance Request
Merrill Engineers and Land Surveyors performed a site inspection of the property on July 30, 2025.
The site was found to be in substantial compliance with the Order of Conditions (83-1352) and approved
Site Plans. The work included the construction of a new dwelling with decks, driveway, patio, utilities,
landscaped areas, and grading.
Minor deviations from the original approved site plan were noted during our site visit and are listed
below:
1) Various walkways and landscape retaining walls with landscape areas surrounding the house
2) A shed was constructed to the south of the existing house
The above -mentioned minor deviations were constructed years ago on the landward side of the limit of
work and no additional clearing was performed. The minor deviations have had no adverse impacts on
the adjacent wetland or the buffer zone associated with it.
We ask that the Conservation Commission issue the compliance request for the above refenced Order.
