Dear Members of the Commission:
On behalf of the Cape Cod Hospital, ILEX Environmental, Inc. (ILEX) is requesting a Certificate of
Compliance for supplemental restoration of wetlands in Yarmouth, Massachusetts performed in
accordance with the Order of Conditions issued on September 7, 2000 (MassDEP File No. 83-
1525). This work supplemented the previous restoration work that was approved on August 13,
1997 (MassDEP File no. 83-1415).
The Order of Conditions approved the continued restoration and enhancement of 18.5 acres of
previously-farmed bordering vegetated wetlands as described in the September 27 .1999
Conservation Plan prepared by the Natural Resources Conservation Service (NRCS) under the
Wildlife Habitat Incentive Program (WHIP). This plan included the following work:
• Restoration of portions of former cranberry bogs (removal of sand/fill; seeding; mowing)
• Construction and planting of wetland shelf in North and South Reservoirs
• Invasive plant control (4.5 acres)
• Boardwalk construction
• Upland buffer zone restoration (removal of sand piles; seeding with native warm season
grasses and wildflowers; mowing)
The only work that was not completed was the construction of the boardwalk. A previous Request
for Certificate of Compliance was submitted on July 14, 2006 but it is unclear why this Certificate
was never issued at that time although it appears that the Request described the work approved
under the 1997 Order of Conditions (MassDEP File No. 83-1415).
We have enclosed the filing fee of $150.00. In addition, we have attached the following documents:

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
Office (see http://www.mass.gov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office-
for-your-city-or-town.html).

Certificate of Compliance
General Information:
•All filings must be made on Town of Yarmouth forms.
•All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
•Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
•Contact the Conservation Office if you need assistance.

office in Town Hall.
