(ILEX) is requesting a Certificate of Compliance of the Order of Conditions issued on May 16, 1979 (MassDEP File No. 83-193). The Order of Conditions allowed work “to erect a tent for Hospital Carisma Night, a fund raising event on lands owned by Cape Cod Hospital, east of Bay View Street…” This Order was issued over 46 years ago and allowed temporary activities for a one night event. As such, we request that the Yarmouth Conservation Commission issue an administrative Certificate of Compliance for this Order of Conditions to close out this file. We have enclosed the filing fee of $150.00. In addition, we have attached the following documents: • Request for Certificate of Compliance Form • Administrative Checklist • Waiver of 21 Day Requirement We look forward to working with the Yarmouth Conservation Commission to obtain the Certificate of Compliance for this project and close out this file.
