L S i -e en slyer _ Map/Lot (?7 !'W3 CIA # 035 -A Teflo Approval Date:c�6/a�,Z,_)/' I certify that I understand the following requirements regarding any changes that may be required for this project: In accordance with paragraph 2(a) of section 1.03(General Procedures) of the OKH 972 CMR Rules and Regulations: Only minor changes may be approved by the Committee without the filing of a new application and a new hearing. Minor changes include alterations that can be done without a detrimental impact on the overall appearance of the project such as altering a single window or door change or a minor change of colors. All minor changes by amendment will require the local Committee's or its designee's approval. All changes to previously OKH approved plans require notification to and approval from the local OKH Committee. Change requests must be submitted to the Committee in writing on the appropriate request form, which may be obtained from the OKH office. All change approvals must be obtained before incorporating the change into the project. If the change has been implemented prior to receipt of OKH approval, a Minor Change approval or Certificate of Appropriateness application for the revised plans is still required and will result in a doubled filing fee for the appropriate category of work. Failure to comply with the above statements will result in the Building Department issuing a stop -work order or delaying the issuance of an Occupancy Permit or final inspection approval. In consideration of total system cost of $36,404, and contingent upon solar loan approval, the parties agree as follows: The Service e 1) Professional�y evaluate the site for oplrmum system performance, structural integr.ty of the mounting area and compatibility of electrical system for interconnect, 2) Prepare, submit and complete all rebate applications. Facilitate all permitting, system interconnection, and acquire PE stamps as necessary. 3) Professionally prepare site, purchase and install a solar photovoltaic system [ma --or components described under System Speaf:cations) including all wiring, conduit, and disconnects to comply with the National Electric Code of 2020, and all appl cable State and Local building codes and laws. 4) Complete installation within 90 days of second payment and receipt of Utility Terms + Conditions, Schedule Z, Electr'c Oil I, and building permit affidavit if applicable. 5) Commission and verify system Is working to specifications. Educate owner about system operation and maintenance. 6) Warrant all workmanship for a period of 10 years from the date of completion, except for acts of God beyond the control of the Service Provider. Instal'ed components have manufacturers warranties. Manufacturers product warranties do not include labor for replacement of parts. Cotuit Solar will cover labor on warrantied parts for 5 years after contract date_ The OwUjr ogre" Igo 1) Make a NON REFUNDABLE deposit payment of $1,500 on gross cost of $36,404 prior to Catuit Solar doing any permitting work Cotuit Solar may void this contract if the deposit is not received within 75 days of Owner's signature. 1) Owner is responsible for ensuring that payment of $36,404 is made per lenders disbursement schedule 2) If owner is paying any portion of project out of pocket, 7096 of total project cost, less deposit amount is due -2 weeks prior to installation. Any remainder of out of pocket cost owed is due within 10 days of completion of installation. If deposit is included in total loan amount, deposit will be refunded once final payment is received, 4) Pay interest of 1.5% monthly on all overdue payments_ 5) This contract does not cover costs for correcting any electrical or budding code violations that exist prior to the installation, or for any upgrades required by utility for PV system interconnection. Any structural upgrades required by the engineer will be an additional cost, or any structural upgrades required by the engineer. 6) That installer may make a recommendation, but ultimately, it is the customers responsibility to determine if they should replace their roof prior to solar .nstallation. We are not a roofing contractor. 7) That Instal ler Is permitted to use photographs of solar installation includi ng system sire and town for marketing purposes 8) Monitoring system production is owner's respons, bility. 9) That online data monitoring portal requires :nternet connection. Costs associated with internet connectivity or communications equipment (if applicable) are not covered by this contract' Any costs associated with connecting the PV system to the internet are subject to billing at hourly rate with Owner approval prior to commencement of work. Any internet connectivity, equipment or communications have a 6 month warranty, after which only equipment warranties as offered by manufacturers app}y. Any time or materials required to fix or replace equipment or correct internet connectivity issues are not covered by a labor warranty after 6 months. For best internet connectivity, we suggest a direct wire to the onsite modem Problematic internet setups that could cause issues and cost you money are (but not limited to)- Mesh networks, Apple modem and router products, and certa n xfnity router and modems. 10) Provide clear physical access to work areas and to adhere to any agreed scheduling of work. Time spent clearing work areas, accommodating last minute schedule changes or having to return to a site due to access or scheduling issues are subject to hourly bill ng at $80 /person hour. 11) System redesign due to changes in Budding, Electrical or Fire codes, ull;ity requirements.
