I certify that I understand the following requirements regarding any changes that may
be required for this project:
In accordance with paragraph 2(a) of section 1.03(General Procedures) of the OKH 972
CMR Rules and Regulations: Only minor changes may be approved by the Committee
without the filing of a new application and a new hearing. Minor changes include
alterations that can be done without a detrimental impact on the overall appearance of
the project such as altering a single window or door change or a minor change of
colors. All minor changes by amendment will require the local Committee's or its
designee's approval.
All changes to previously OKH approved plans require notification to and approval
from the local OKH Committee. Change requests must be submitted to the Committee
in writing on the appropriate request form, which may be obtained from the OKH office.
All change approvals must be obtained before incorporating the change into the
project.
If the change has been implemented prior to receipt of OKH approval, a Minor Change
approval or Certificate of Appropriateness application for the revised plans is still
required and will result in a doubled filing fee for the appropriate category of work.
Failure to comply with the above statements will result in the Building Department
issuing a stop -work order or delaying the issuance of an Occupancy Permit or final
inspection approval.

Re: Request Work To Be Done At 129 Driftwood Lane In §W9W*hW loi&d King's Highway
Historic District Historic District
Front of the house I would Like To add 3 sets of dormers across the front of the house as seen
on the attached plans.
A small overhang at the front door entryway stairs will remain just like adding a small roof as
shown on plans.
All the siding will be removed from the house on all 4 sides keeping with the existing home. I will
install new Hardy cement clapboards to the whole front facing house and the other 3 sides will
have new white cedar shingles to replace the existing ones.
Color will be a light grey color as the house is now. And the white cedar shingles will remain
natural and will fade grey naturally. All trim boards will be removed and replaced with White
Azek trim boards.
The existing screen porch of the left side of the house will remain the same size and look just
replaced with new screens and Azek trim boards and a Light Grey coastal color Azek decking.
As you can see from the pictures I provided behind the house is a small wrap -around deck on
the left side and part of the rear of the home. I would like to remove that deck and only replace it
with a set of stairs off the screen porch on the back side of the porch. The rear part of the deck
will be removed and I would like to add a small porch with a set of stairs to ground from the rear
slider.
All the windows in the home will be replaced with new Anderson Series 400 windows white clad
on exterior and wood interior with a grill pattern 6 over 6.
i would like to remove the existing front door that no longer opens and closes and replace it with
a new Fiberglass exterior door with a 6 Lite pattern at the top part of the window and raised
panels on the lower part of the door.
The existing asphalt Architectural roof shingles will be removed and I would like to Install new
Architectural roof shingles in Black or charcoal color.
Garage Doors: I would like to replace the 2 existing damaged garage doors with New garage
doors, the same raised panel galvanized white metal insulated doors.
The existing Asphalt driveway I would like to remove and replace with a shell driveway.
The whole yard will be relandscaped existing beautiful trees will remain but trimmed back.
