This notice of violation is issued to the property located at 411 Route 6a in
YarmouthPort, Mass. During a routine safety inspection of the building performed
on October 22,2014 at l330hrs. I observed many deficiencies at the different 7
buildings on the property. I was not able to gain access to all portions ofbuildings
1,2,3,4,5,7.
I was able to perform an inspection of building 6, Habitat for Humanity. I found
the Fire Alarm System in trouble at the panel, I tested the manual pull station
located at the main doorway, and found the system not functioning. This is a
violation of 527 CMR l:06 (2) which states The fire alarm system must be
maintained.
Corrections must be made immediately, contact my office as soon as possible to
arrange a time for a complete inspection of all buildings.

Lieutenant Bearse,
As discussed, lntercity Alarm made the necessary fire alarm repairs to the building occupied by
Habitat for Humanity at 411 Main Street Yarmouth Port. They will also be checking the other
buildings in the next couple of days when doing their Annual Fire Alarm lnspection and making any
necessary repairs if needed. I will notify you when this work is completed and we can inspect the
rest of the property at a time convenient for you. lf you have any questions in the meantime please
