Dear Commissioners:
S/e have inspected the above property under its order of conditions and request a
'Certificate of Compliance' from you. Attached is an as-built plan dated today.
The shaded areas adjacent to the foundation are stone fienching, which is in place under
the drip areas. The driveway shape has changed from proposed, but it is pervious. The
east wall was modified from proposed plan to accommodate the existing gate through the
'Radio City' perimeter fence (fence not shown).
The west wall was built longer than shown on the revised plan dated l2l2l/18, but the
wall built is not inconsistent with the Conservation Minutes from the day before.
Please read these minutes. Thank you.
We find the locus property to be in significant compliance with the Order of Conditions
and request the Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
