the above referenced property in West Yarmoutb Massachusetts. GSE respectfully
requests a 21 E file review from the Yarmouth Fire Department for the subject property
and any surrounding properties that may present a Recognized Environmental Condition
to the subject property, pursuant to ASTM 1528-00.

Dear Mr. Flood;
Here is your request for a 21E fi1e review for the prcperty located at 303 Route 28 West
Yarmouth MA. Klown as Yarrnouth Physical Therapy.
I have researched our records for any UST's, and AST's, and found we have no record ofany
at this property or any adjoining properties.
I do remember that the previous owner ofthis property ran an extennination company out of
this building and used the basement for the storage of chemicals, but we have no record of any
probiem at this site.
There have been a number ofminor oil and gasoline spills do to auto accidents on the comer of
Rt.28 and Standish Way, but none with any impact on the environment.
