formed on the Customer's equipmenl by a Representative of Ralph J. Perry, lnc. will indicale that
the Fire Suppression system was mechanically functioning during lhe period of time rn which
Ralph J. Perry, lnc. Representative was performing said service. The Customer acknowledges
that Ralph J. Perry, lnc. does not guarantee, imply or suggest that the Customer's Fire Suppres-
sion System will extinguish all fires regardless of origin. The Customer further acknowledges
that Ralph J. Perry, lnc. shall have no responsibility whatsoever to the Customer or to any other
person for personal injury or death or damage to or loss of property or value, resulting from any
causes beyond reasonable control, including, without limitation. if the Fire Suppression System
is outdated, has been tampered with, altered or has been improperly used, repaired or main-
tained, or if the hazard areas protected by the Fire Suppression System has been altered or
changed.

RE: Affidavit stating use of Small Stove in Concession Stand
As per our conversation on Sat. 4/7106 in regards to a small stove having to be placed
under the Exhaust Hood and Ansul System OR NOT, I assure that no cookfurg of grease,
fried foods or readily combustible ingredients will be cooked or prepared on this STOVE.
It's sole purpose is for warming and small quantity boiling.
