declaration of trust dated December 21, 1999, and filed with the Barnstable Registry District of
the Land Court as Document No. 820,791, of 329 Main Street, Centerville, Massachusetts

for no consideration and as a distribution to the beneficiaries of said trust upon termination of
said trust, pursuant to the terms thereof, grant to
John A. Vellone, Jr., Trustee of The John A. Vellone, Jr. Revocable Trust, under declaration of
trust dated December 15, 1995, a Trustee's Certificate for which is filed with the Barnstable
Registry District of the Land Court herewith, as to a fifty (50%) percent undivided interest, and
Judith A. Vellone, Trustee of The Judith A. Vellone Trust, under declaration of trust dated
October 27, 1992, a Trustee's Certificate for which is filed with said Registry District herewith,
as to a fifty (50%) percent undivided interest, as tenants in common, both of 552 Shoot FIying
Hill Road, Centerville, Massachusetts 02632,
the land, together with the buildings thereon, situate in Yarmouth (West Yarmouth), Barnstable
County, Massachusetts, more particularly bounded and described as follows:
Being shown as Lot 9 on Land Court Plan No. 28808-D.
For title, see Certificate of Title No. 160230.
