To whom it may concern,
The Building Depaffnent conducted a_site visit after being notified ofpotential unsafe conditions at the property.During the course ofthe inspection of the exterior, there-were a few iiems that are.on...ln!r. ir,"y relate to the safetyand security ofthe occupants as well as the safety of Emergency Responders.The front porch is in bad condition and appears unsafe as its sutctural integrity is in question. There are several windowsopen or broken.

The Building Department requests entry into this dwelling under section Rl04.6 ofthe Massachusens state Building code.

You are hereby directed to have a licensed professional submit a permit application to make the necessary repairs as notedin this notice, as well as any others identified after completion olan inspection. All necessary .epairs *iil be inspected bythe Building Department.
