Mr. Roy,
A recent inspection at the above property revealed a number of items that require your
aflenlion for correction or repair.
First and foremost, the electricity service drop must be replaced by a licensed Electrician
due to deterioration exposing the neutral connection. This may be the reason that some
ckcuits or switches are problematical. It also presents a potential hazard.
Exterior:
I Replace electrical service drop to meter. Permit and inspection required.

the same time. More than one switch plate found to be covered by black
electrical tape.
Install screen doors at Living room and Kitchen entries. (410.552)
Install window screens for all openable windows. Temporary, expandable
screens are not considered acceptable. (410.551)
(screens are required to be in place from April lst to October 30s)
No working Smoke Detectors were found. Detectors in the dwelling had no
batteries and should be replaced with unit due to age. It is suggested that
combination Smoke/Carbon Mo ts be installed. Either battery
operated or hardwited./battery

The above are noted as violations ofthe State Sanitary Code, 105 CMR410.000, Chapter
II: Minimum Staadards of Fitness for Humal Habitation. Corrections must begin
immediately and be substantially completed within not more than 30 days.

accordance with Town by law Chapter 108. A form
