Elliott, Inspector of Wires kelliott@varmouth.ma.us INCIDENT REPORT September 5, 2012 Time of Call:1715 Hrs Address:39 Anchorage Lane George Slack On this date I was requested, by Yarmouth Fire Department, to respond to a reported down wire at the above address. Upon arrival I met with members of the F.D. and was shown the area of concern. There was a drop wire from the pole to the residence that was burnt off and separated. The section, still connected to the house was lying on the ground. The live section, still connected at the pole, was still hanging in the tree. This is possibly lightning damage but a higher probability that the drop wire was chafing in the tree limbs and finally burnt through. I advised the owner to stay clear of the area and that NSTAR had been notified to respond and that they would make it safe and replace the drop wire. I did not observe any further damage in or about the property. Respectivelv submitted {futA K. Elliott Inspector of Wires Town of Yarmouth, Building Department Date: Name:
