In March 2022, the Commission issued a Stormwater Management Permit for the
redevelopment of the above -referenced site. The scope of the redevelopment included
the renovation of the existing buildings generally within their existing footprint.
Additionally, the reconstruction of the existing parking lot to meet updated parking
requirements in accordance with current regulations.
Throughout the construction process, multiple site visits were conducted to confirm the
proper installation and location of all stormwater and drainage structures, all of which
are accurately shown on the final as -built site plan. Erosion and sediment control
measures were actively maintained during construction to prevent discharge, and all
stormwater structures were installed in accordance with the approved utilities site plan.
All required inspections were completed in compliance with the Special Conditions
outlined in the Stormwater Management Permit.
As required under the permit, the following ongoing stormwater maintenance conditions
are being complied with:
• Bi-annual sweeping of all parking areas.
• The owner shall provide quarterly inspections of all catch basins and retain
records. Catch basins shall be cleaned once they reach 50% sediment depth or
once every two years, whichever occurs first.
This is to certify, to the best of my knowledge and belief, that all work has been
completed in substantial compliance with the approved Stormwater Management Permit
and its associated conditions. Based on this, we respectfully request a formal closeout of
the Stormwater Management Permit for this site.

RE: #125)-133 Route 6A
Down Cape Engineering proposes to do the required inspections based on the
Stormwater O&M Plan and Stormwater Permit recorded in the Registry of Deeds.
The scope of work will be to physically measure the sediment depth in all the onsite
catch basins, inspect the dumpster covers, and provide a report to the Yarmouth
Conservation Department summarizing the findings. If sediment depths are over two
feet in thickness, we would recommend a catch basin cleaning company be retained
by UACC for maintenance.
The O&M plan we prepared recommended 2x per year, but the Permit they issued
required 4x per year. I would suggest hiring us to have a meeting with the
Commission to request reducing the scope to 2x per year after a year of observations.
Our fee for each visit, now four times per year, would be )er visit/report.
We can hold this fee level for the three years of contract length, whether 2 or 4 visits
per year. The bi-annual parking lot street sweeping is not included in our fee, nor
basin cleaning if required.
If this is acceptable please sign below as an acknowledgement and we will invoice you
for each visit within a month of taking the readings, terms net in 30 days.
