OPERATION AND MAINTENANCE PLAN SUMMARY
The O&M Plan for this property includes the following maintenance activities:
Quarterly Inspections:
• Report on depth of sediment in stonnwater catch basins
• Check dumpster covers to ensure they are secure and prevent contaminated runoff
• Submit inspection reports to the Town
Annual Spring Maintenance:
• Sweep parking lot on or about April 1st to remove winter debris and sand
accumulation
COMPLIANCE CERTIFICATION
I, the undersigned, as the owner/authorized representative of the above-referenced property,
hereby submit this Operation and Maintenance Compliance Statement in reference to
Condition 19 located in the General Conditions section of the Stonnwater Management Pennit
and certify the following:
l. Operation and Maintenance Plan Completeness:
The Operation and Maintenance Plan ("O&M Plan") for all stonnwater Best
Management Practices (BMPs) installed on the subject property is complete and will
be implemented immediately upon receipt of the Certificate of Completion from the
issuing authority.
2. Notification of Future Responsible Parties:
All future responsible parties will be notified in writing of their ongoing legal
responsibility to:
o Operate and maintain the stonnwater management BMPs in accordance with
the O&M Plan
o Implement the Stonnwater Pollution Prevention Plan
o Complete all required quarterly inspections and submit reports to the Town
o Perfonn annual spring parking lot sweeping as outlined in the O&M Plan
3. Assumption of Responsibility:
I acknowledge that as the landowner, I am responsible for BMP maintenance until the
issuing authority is notified in writing that another party has legally assumed
responsibility for BMP maintenance.
4. Compliance Commitment:
I certify that the responsible party identified above will ensure proper operation and
maintenance of all stormwater BMPs in accordance with applicable regulations and the
approved O&M Plan.
