On behalf of our client, Bayridge Realty LLC, we submit this VCOD Site Plan Review application, with site
plans, architectural floor plans, elevations and renderings, and the $350 administrative filing fee. The
project proposes to demolish the existing restaurant building, and redevelop the site with one two-story
mixed -use building, with new paved parking lot, drainage systems, lighting, landscaping, and
underground utility services. The building is proposed as two commercial units at the front of the first
floor, and apartment units at the rear of the first floor, and on the second floor.

Narrative for VCOD SPR Application — 645 Main Street West Yarmouth MA
Bayridge Realty LLC recently purchased the property at 645 Main Street (Route 28) in West Yarmouth,
MA. The property consists of two parcels — Map 32, Parcels 136 and 137, which comprise approximately
0.94 acre. The site lies in a split zone, B-2 to the north, and R25 to the south. The B-2 portion also lies
within the Village Center Overlay District, as such, requires the VCOD Site Plan Review process.
The property is subject to a taking by MassDOT for Layout Alteration #8854, as well as an easement, per
MassDOT Project File No. 608264, and these are shown on the site plans.
The site is currently improved with a two-story building, which has been used for restaurant use. The
building is currently vacant, and the site is ripe for redevelopment. The wooden structure is dilapidated
and will be removed. The site currently exists with the two-story wooden structure, an overgrown
parking lot, with landscaping on the exterior of the parking areas. A stand of deciduous trees lies along
Appleby Street towards the south part of the site. The trees will largely remain, with some removed for
the proposed driveway off Appleby Street, and some cleared for the required site distance.
The proposed use of the site is for mixed use, consisting of two commercial units on the first floor facing
Route 28, and the remainder of the first floor will be 5 multi -family housing units. 6 multi -family housing
units are proposed for the second floor.
Access to the site is from one driveway, off of Main Street. The Mian Street curb cut is proposed to be
closed with this redevelopment project, and a new driveway is proposed off of Appleby Street. The
driveway modifications are subject to a permit from MassDOT. The parking lot is paved, but will be
removed as the site will be regraded, with the building floor elevation raised to more than 2 feet above
the flood elevation of 11 feet.
The site will be served by underground electric, telephone and data services from a utility pole on
Appleby Street. The site will be served by an existing natural gas service off of Appleby Street.
Domestic water service and a fire suppression service will by served by the existing water main on
Appleby Street. A new main will be installed in Route 28 as part of the sewer installation, but the water
dept that tapping the services off of the Appleby main will be allowed. Wastewater at the site will flow
to a new sewer service from Appleby Street, proposed to the installed by the Town as part of the Phase
sewer installation by the Town. A grease trap is proposed to removed kitchen flow grease prior to
discharge to the sewer service, as the Board of Health requires.
Stormwater at the site will flow thru a curb opening into a sediment forebay where pollutants will be
filtered out. When the forebay begins to fill up, stormwater will then flow into a beehive grate catch
basin, then recharge into the groundwater through underground Cultec infiltration chambers. Roof
runoff will be collected in gutters and flow to the underground Cultec system with direct connections
through CPP piping. Stormwater calculations are included with this application. Existing and proposed
wastewater calculations are also included with this application.
The project is proposed to be constructed in one single phase.

Narrative for VCOD SPR Application
Zoning Bylaw 412.2 -- Inclusionary Zoning — requires 2 affordable housing units in a development of 11
housing units. 2 affordable units will be provided on site as 2 of the 11 housing units.
All site maintenance will be managed by a property manager who will be an employee of Bayridge Realty.
This person will be responsible for all building and property maintenance, as well as solid waste
management, landscaping maintenance, and stormwater maintenance. Funding for maintenance staff
will be paid by Bayridge Realty.
Areas of the site not covered by building or parking will be landscaped.. Landscaping maintenance will be
provided by Bayridge Realty. Two bike rack areas will be installed as part of the site development, to
encourage employees biking, and reduce the number of vehicle trips.
