Dear Ms. DiRienzo,
Please accept this Request for a Certificate of Compliance on behalf of our client, Valentina Karcha,
for the project authorized in the Order of Conditions (DEP File #SE 083-2310) for the completed work
at 52 Wilfin Road in Yarmouth (the Site). The completed work was issued an Order of Conditions on
December 23rd, 2021 and expired on December 23rd, 2024. Construction and an as-built survey were
completed in September 2022. The approved project consisted of the razing and replacement of a
single-family home. This project has been completed in significant compliance with the order of
conditions verified by a site visit in Summer 2025.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
Office (see http://www.mass.gov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office-
