Refer to Meeting Schedule. • Failure to follow the Administrative Checklist shall result in an Administrative Incomplete Application and will not be scheduled for a Public Hearing. • Contact the Conservation Office if you need assistance. Submitting an Application: Hardcopy of the COC application mailed to or dropped off at the conservation Office In Town Hall. Signed statement from an Engineer or similar professional who prepared the approved plan stating the date of a recent site inspection and certifying compliance with the permit and outlining departures from the approved plan, if any exist. If mitigation planting was required, include planting date and confirm the required time has elapsed*. As-Built plan – *Refer to issued Order of Conditions: Special Conditions to determine if required*. CONSERVATION OFFICE X X X X N/A N/A December 31, 2025 Brittany DiRenzo, Administrator Yarmouth Conservation Commission 1146 Route Yarmouth, MA 02536 RE: Request for Certificate of Compliance 22 Driftwood Lane, Yarmouth, MA 02664 DEP SE 83-1153 Dear Chairman & member of the Yarmouth Conservation Commission: Per request of the homeowners, Richard and Carol Dove, Cape & Islands Engineering submits this request for a Certificate of Compliance for the work completed in accordance with the Order of Conditions (OOC) DEP File No. SE 83-1153 issued on December 5, 1991 for Robert E. Romano. Based on a site inspection completed December 9, 2025, the project has been substantially completed according to the plan of reference titled, “Plan of Land in Yarmouth, Massachusetts for Robert E. Romano” dated November 19, 1991, prepared by Robin W. Wilcox, P.L.S. The project was constructed in substantial compliance with the approved plan and Order of Conditions. The property has another Order of Conditions 83-1726 issued August 22, 2005. The site was determined to be conforming, and a Certificate of Compliance was issued on October 2, 2015. Did the Order of Conditions for this project, or the portion of the project subject to this request, contain an approval of any plans stamped by a registered professional engineer, architect, landscape architect, or land surveyor? No B. Submittal Requirements Requests for Certificates of Compliance should be directed to the issuing authority that issued the final Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit this request to that Commission.
