Dear Ms. DiRenzo and Members of the Commission,
I am requesting a certificate of compliance be issued for 34 Lyndale Road in South
Yarmouth on behalf of the owners, Frederick & Dawn Brundage. The Order of
Conditions was issued on April 13, 2022. The project appears to be in substantial
compliance with the order of conditions with the exception of the front and rear stoop.
The front stoop is a little bigger and the rear stoop is a little smaller than depicted on the
approved plan. Additionally a shed was built on the rear of the property. The dwelling
was also constructed in compliance with the flood zone requirements of 2005.

Signed statement from an Engineer or similar professional who prepared the
approved plan stating the date of a recent site inspection and certifying compliance
with the permit and outlining deparfures from the approved plan, if any exist.
If mitigation planting was required, include planting date and confirm the required
time has elapsed*.

8. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this,request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
