Did the Order of Conditions for this project, or the portion of the project subject to this request, contain an approval of any plans stamped by a registered professional engineer, architect, landscape architect, or land surveyor? Yes If yes, attach a written statement by such a professional certifying substantial compliance with the plans and describing what deviation, if any, exists from the plans approved in the Order. No B. Submittal Requirements Requests for Certificates of Compliance should be directed to the issuing authority that issued the final Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit this request to that Commission. If the project was issued a Superseding Order of Conditions or was the subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional Office (see http://www.mass.gov/eea/agencies/massdep/about/contacts/find-the-massdep-regional-office- for-your-city-or-town.html). January 5, 2026 Attn: Brittany DiRienzo 1146 Route 28 South Yarmouth, MA 02664-4492 Re: Request for Certificate of Compliance and Statement of Compliance SE83-856 Garage Addition 41 Standish Way West Yarmouth Please accept this request for a Certificate of Compliance for the above referenced project. The project was approved by the Conservation Commission back in December 1987 for Charles Hennessey, the owner at the time. The property was then purchased by Martin, Gerald and Margaret Dugal in 2013, now the current owners. When purchased in 2013, the garage addition work was already completed. This request is to close the still open permit. A site inspection was completed by Martin Dugal, PE, On November 8, 2025. The project was constructed substantially in accordance with the Sketch Plan of Land of December 1, 1987. There is one deviation to note. 1) Where the plan shows an angled cut corner in the western corner, the contstructed addition is rectangular. (see plans) Attachments include: - Approved sketch plan (1987) - Plot Plan (2013) - As constructed plan (2025) The above mentioned change does not substantially affect the project. Please include this request in your next public hearing. The original COC should be mailed to 41 Standish Way for recording.
