" To: Ken Elliott Cc: Mark Grylls From: AJ Pulley Date: December 22,2021 Re: Yarmouth P.D.HVAC Project Ken, Josh Burgen,the on-site electrical foreman for the HVAC project at the Y.P.D.,requested a courtesy visit for concerns relating to existing violations he has found as a result of previous work by others.During my visit with him,he showed me various issues,such as open/uncovered junction boxes, open/uncovered conduit LBs and unsecured MC cables. While I cannot require correction of work not performed by this contractor,I suggested he bring it to the attention of the Clerk of the Works on behalf of the Town to address this accordingly for reasons of safety for approval from representative persons of the Town. Additionally,I asked Mr.Burgen to start and maintain a ledger of these issues,whether current or any found thereafter,to include the nature of the issue he has found,along with the date and a brief description of where the issue is located. This is to be submitted to the Building Department upon his request for final inspection/job completion. Secondly,there is an HVAC unit in a first floor corridor ceiling,located above architectural tiles,outside the exercise room. Due to its location,and its vicinity to numerous other systems located in close proximity thereto,the contractor proposed installing an access panel in the demising wall between the exercise room and corridor to allow access to the electrical compartment of the HVAC unit. I feel this is reasonable due to the unique circumstances,and therefore I am require the following: 1. The disconnecting means for the unit must be located at the access panel in the exercise room, and 2. The disconnecting means be of the lockable type for application of a lock-out/tag-out device, and 3. Signage be placed on the corridor wall below the unit indicating the location of the disconnecting means. Lastly,I asked Mr.Burgen if any duct smoke detectors must be replaced or relocated as a result of ducting changing with respect to the manufacturer's installation requirements where in conflict with distances from bends in duct work,that this be specifically included in a future electrical inspection and pointed out to you with respect to wiring.
