Re: Cape Cod Hospital - Cancer and Cardiology Expansion

Dear Jordan:
This office represents Cape Cod Hospital (hereafter, "CCH"),owner of the
approximately 17-acre property addressed 27 Park Street, Hyannis, where the Cape Cod Hospital
is situated. As described in the attached materials, CCH proposes an addition to the southwest
corner of the hospital consisting of four patient floors and a rooftop mechanical penthouse. The
addition contains approximately 137,051 square feet of gross floor area and includes a new
cancer center(2 stories, first and second floors), a third floor with 32 cardiovascular
medical/surgical beds, a fourth floor which will initially be shell-space but available for future
beds or clinical areas, and a ground floor surgical expansion infill area that will connect the
addition to the main hospital building, together with infrastructure and site improvements (the
"Project"). CCH is committed to providing the highest level of care to the community and this
very important Project will enhance existing services and provide a place of restorative health for
patients, Cape Cod Hospital staff, and the community.

In support of this filing, I enclose:
1. One (1) hard copy of the application notebook;
2. One (1) full size copy of the Project plans (reduced sized copies are contained in
Tab 24 of the application materials); and
3. Certified abutters lists from the Town of Barnstable and Town of Yarmouth,
together with three (3) sets of mailing labels.

I will provide a thumb-drive with all the application materials under separate cover. I also
note that the filing fee calculation and a request for reduction of the filing fee is included in Tab
8 of the application materials.
Per the Cape Cod Commission requirements, copies of this filing are also being provided
to the Towns of Barnstable and Yarmouth, and we will be working with the Town of Barnstable
to obtain a referral for the Project.
