11.LOCATIONS OF EXISTING SUBSURFACE UTILITIES SHOWN ON THESE PLANS
ARE BASED UPON RECORDS AVAILABLE TO THE ENGINEER AND ARE NOT

15.DURING OPERATIONS WITHIN PUBLIC RIGHT-OF-WAY, THE CONTRACTOR
SHALL EMPLOY TRAFFIC CONTROL AND TEMPORARY CONSTRUCTION
SIGNING ARRANGEMENTS IN ACCORDANCE WITH LOCAL AND STATE
REQUIREMENTS.
16.SNOW STORAGE AREAS ARE SHOWN ON THE SITE PLAN. ANY SNOW IN
EXCESS OF THE STORAGE AREAS SHOWN WILL BE REMOVED AND DISPOSED
OFF PROPERTY.
17.CONTRACTOR TO MAINTAIN ACCESS FOR EMERGENCY VEHICLES AT ALL
TIMES.
18.THE CONTRACTOR SHALL ADHERE TO ALL PERMIT CONDITIONS ISSUED BY
LOCAL, STATE, AND FEDERAL AGENCIES.
19.AN AS-BUILT SHALL BE SUBMITTED UPON OCCUPANCY FOR ALL WORK ON

20. ALL BOARD OF HEALTH PERMITS MUST BE OBTAINED PRIOR TO OPERATION.
21.THE PROJECT REQUIRES RELIEF FROM THE FOLLOWING SECTIONS OF THE
ZONING BYLAW:
·SECTION 301.8: "ANY OUTDOOR SERVICE FACILITY (INCLUDING MENU
BOARDS, SPEAKERS, ETC.) SHALL BE A MINIMUM OF 100 FEET FROM ANY
RESIDENTIAL USE OR ZONE.
·SECTION 301.4.3: "DRIVEWAYS SUBJECT TO THIS SECTION SHALL HAVE
TWO HUNDRED FIFTY (250) FEET VISIBILITY IN EACH TRAVEL DIRECTION
AND SHALL BE COMPRISED OF TWO (2) TRAVEL LANES EACH NOT MORE
THAN TWELVE (12) FEET NOR LESS THAN TEN (10) FEET IN WIDTH; A
MINIMUM CURB RADIUS OF 25 FEET SHALL BE PROVIDED."
·SECTION 301.4.1: "PARKING AREAS SHALL BE LOCATED AT THE SIDE
AND/OR REAR OF A PRINCIPAL STRUCTURE(S), UNLESS THE SITE PLAN
REVIEW TEAM DETERMINES THAT THE ALREADY DEVELOPED SITE WORK
MAKES SUCH PLACEMENT IMPOSSIBLE. PARKING ON THE SIDE OF
STRUCTURES SHALL BE LOCATED BEHIND THE MINIMUM FRONT YARD
SETBACK SHOWN IN SECTION 203.5 – TABLE OF DIMENSIONAL
REQUIREMENTS. APPLICABLE PROPERTIES IN THE HMOD2, AS DEFINED IN
SECTION 404.2.2, HAVING PROPERTY LINES ABUTTING NANTUCKET SOUND
MAY LOCATE PARKING AREAS TO THE FRONT OF THE STRUCTURE(S). IF
NECESSARY, HANDICAPPED PARKING MAY BE LOCATED TO THE FRONT OF
A BUILDING IN ORDER TO COMPLY WITH 521 CMR, THE RULES AND
REGULATIONS OF THE ARCHITECTURAL ACCESS BOARD.
·SECTION 202.5: SPECIAL PERMIT FOR AN EATING AND DRINKING
ESTABLISHMENT IN THE AQUIFER PROTECTION DISTRICT (APD).
DEMOLITION NOTES:
1.DEMOLITION ACTIVITIES SHALL ADHERE TO ALL FEDERAL, STATE, AND LOCAL
REGULATIONS. EROSION AND SEDIMENTATION CONTROL MEASURES MUST
BE INSTALLED PRIOR TO THE START OF DEMOLITION.
2.THE CONTRACTOR IS RESPONSIBLE FOR ENSURING THE SAFETY OF ALL
PERSONNEL AND PREVENTING DAMAGE TO ADJACENT STRUCTURES AND
SITE IMPROVEMENTS DURING DEMOLITION.
3.THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING A DEMOLITION PERMIT
FROM THE MUNICIPALITY PRIOR TO COMMENCEMENT OF THE WORK.
4.DEMOLITION ACTIVITIES SHALL BE CONDUCTED IN A MANNER TO ENSURE
MINIMUM INTERFERENCE WITH PUBLIC ACCESS AREAS INCLUDING ROADS,
SIDEWALKS, AND ADJACENT FACILITIES. PROVIDE ALTERNATE ROUTES
AROUND CLOSED OR OBSTRUCTED TRAFFIC WAYS.
5.CONTRACTOR SHALL NOTIFY AND COORDINATE WITH DIGSAFE AT 811 PRIOR
TO ANY EXCAVATION. ALL EXISTING UTILITY DISCONNECTIONS SHALL BE
COORDINATED WITH THE RESPECTIVE UTILITY PROVIDER.
6.USE WATER OR ALTERNATIVE APPROVED DUST CONTROL METHODS TO LIMIT
THE AMOUNT OF DUST IN THE AIR DURING DEMOLITION.
7.EXPLOSIVES ARE PROHIBITED UNLESS OTHERWISE NOTED. PERMISSION AND
WRITTEN CONSENT MUST BE PROVIDED BY THE DEVELOPER AND
AUTHORITIES PRIOR TO USE.
8.ALL VOIDS RESULTING FROM DEMOLITION SHALL BE BACKFILLED WITH
COMPACTED, GRANULAR MATERIAL FREE OF ORGANICS AND DEBRIS.
BACKFILL SHALL BE PLACED IN LIFTS NOT EXCEEDING 6 INCHES AND
COMPACTED TO 95% OPTIMUM DENSITY.
9.ALL DEBRIS AND WASTE RESULTING FROM DEMOLITION SHALL BE REMOVED
FROM THE SITE AND DISPOSED OF IN ACCORDANCE WITH ALL APPLICABLE
REGULATIONS. NO MATERIALS SHALL BE STORED, SOLD, OR BURNED ON
SITE.
10.SHUT OFF, DISCONNECT, AND SEAL ALL UTILITIES SERVING ANY STRUCTURES
INDICATED TO BE DEMOLISHED PRIOR TO COMMENCEMENT OF THE
DEMOLITION ACTIVITY. LOCATE ALL UTILITY, DRAINAGE, AND SANITARY LINES
AROUND THE DEMOLITION. PROTECT ALL ACTIVE DRAINAGE OR UTILITY
LINES. NOTIFY THE ENGINEER 48 HOURS IN ADVANCE OF ANY UTILITY
SERVICE INTERUPTIONS. NOTIFY ALL APPLICABLE UTILITY COMPANIES TO
ENSURE CONTINUATION OF THE SERVICE.
11.FEATURES LABELED ON THE PLANS “TO BE REMOVED" OR “REMOVE AND
DISPOSE” ARE DESIGNATED FOR DEMOLITION. ALL OTHER EXISTING
FEATURES SHALL REMAIN UNLESS SPECIFIED OTHERWISE.
12.CONTRACTOR SHALL PROTECT EXISTING TREES AND PLANTINGS, WALLS,
PAVEMENTS, AND SITE FEATURES NOT DESIGNATED FOR REMOVAL.
13.ANY DAMAGE TO EXISTING ROADWAYS, INFRASTRUCTURE, OR PROPERTIES
SHALL BE REPAIRED AT THE CONTRACTOR'S EXPENSE.
14.ALL EXISTING GRASS, LANDSCAPED, OR VEGETATED AREAS WITHIN THE
LIMITS OF WORK PROPOSED TO BE RECONSTRUCTED AS PAVEMENT,
SIDEWALK, OR OTHER STRUCTURE SHALL HAVE ALL PLANTINGS, SHRUBS,
MULCHES, TOPSOIL, AND ORGANIC MATERIAL REMOVED TO FULL DEPTH
PRIOR TO PREPARING SUBGRADE. TOPSOIL SHALL BE STOCKPILED AND
REUSED IN PROPOSED LANDSCAPE AREAS AND/OR REMOVED FROM THE SITE
UPON COMPLETION.
15.CONTRACTOR SHALL LOCATE AND REMOVE EXISTING SEPTIC SYSTEMS IN
ACCORDANCE WITH STATE REGULATIONS.
GRADING & DRAINAGE NOTES:
1.ELEVATIONS SHOWN ARE REFERENCED TO VERTICAL DATUM NAVD88.
2.CONTRACTOR SHALL VERIFY EXISTING SITE ELEVATIONS UPON
ESTABLISHING CONTROL ON THE SITE. ANY DEVIATIONS BETWEEN EXISTING
TOPOGRAPHY AND THAT SHOWN ON THE PLANS SHALL BE IDENTIFIED AND
COORDINATED WITH THE ENGINEER.
3.ALL PROPOSED STRUCTURES, SIDEWALKS, AND PAVEMENTS SHALL BE
INSTALLED IN ACCORDANCE WITH THE PLAN GRADES, OR AS OTHERWISE
DIRECTED BY THE ENGINEER.
4.ALL PROPOSED ELEVATIONS SHOWN ON THE PLANS AT PAVEMENT AREAS
ADJACENT TO CURBING REPRESENT THE BOTTOM OF THE CURB UNLESS
OTHERWISE SPECIFIED.
5.CONTRACTOR SHALL ENSURE POSITIVE DRAINAGE AWAY FROM BUILDINGS.
6.WHERE PROPOSED PAVEMENT MEETS EXISTING, EXISTING PAVEMENT SHALL
BE SAWCUT AND A PAVEMENT JOINT SHALL BE INSTALLED TO ENSURE A
SMOOTH AND CONTINUOUS GRADE.
7.IN LANDSCAPED AREAS, THE TOP/RIM ELEVATION OF MANHOLES SHALL
MATCH THE FINISH GRADE OF THE TOPSOIL. IN PAVED AREAS, THE TOP/RIM
ELEVATION OF MANHOLES SHALL MATCH FINISH GRADE.
8.ALL PROPOSED CATCH BASINS SHALL BE CONSTRUCTED WITH 4-FOOT
SUMPS. OUTLETS SHALL BE FITTED WITH AN APPROVED OUTLET HOOD TO
PREVENT SEDIMENT MIGRATION.
9.ALL PROPOSED STORM DRAINAGE PIPE SHALL BE DUAL-WALL, CORRUGATED
HDPE WITH A SMOOTH INTERIOR AND STANDARD JOINTS, AS MANUFACTURED
BY ADS, INC., OR APPROVED EQUAL, UNLESS OTHERWISE NOTED ON THE
PLANS.
10.ALL ROOF DRAIN PIPE SHALL BE 6" PVC (SDR-35), EXCEPT WITHIN 10 FEET OF
A BUILDING FOUNDATION WHERE CAST IRON PIPE SHALL BE USED.
COORDINATE WITH PLUMBING ENGINEER AND/OR INSPECTOR.
11.THE CONTRACTOR IS RESPONSIBLE FOR FIELD-ADJUSTING GRADES TO
ENSURE PROPER DRAINAGE FROM ALL SURFACES.
12.ALL ADA ACCESSIBLE WALKWAYS AND CROSSWALKS SHALL NOT EXCEED 5%
RUNNING SLOPE AND 2% CROSS SLOPE. ACCESSIBLE RAMPS SHALL NOT
EXCEED 8.33% RUNNING SLOPE AND 2% CROSS SLOPE. ACCESSIBLE
PARKING STALLS AND ACCESS AISLES CANNOT EXCEED 2% SLOPE IN ANY
DIRECTION. PRIOR TO CONSTRUCTION, CONTRACTOR SHALL NOTIFY
ENGINEER OF ANY DISCREPANCIES.
13.THE CONTRACTOR SHALL COORDINATE THE SCHEDULE AND TIMING OF FINAL
GRADING WITH LOAM AND SEEDING OPERATIONS.
14.PIPE DATA PROVIDED ON PLANS IS CALCULATED TO CENTER OF STRUCTURE.
15.ALL PROPOSED DRAINAGE STRUCTURES SHALL BE CLEANED PRIOR TO FINAL
ACCEPTANCE.
UTILITY NOTES:
1.THE LOCATIONS OF ALL EXISTING UNDERGROUND UTILITIES SHOWN ARE
APPROXIMATE AND BASED ON INFORMATION AVAILABLE TO THE SURVEYOR
AND ENGINEER. IT IS THE CONTRACTOR'S RESPONSIBILITY TO CONTACT DIG
SAFE (DIAL 811) AND THE LOCAL WATER DEPARTMENT AND UTILITY
PROVIDERS AT LEAST 72 HOURS PRIOR TO ANY EXCAVATION TO HAVE ALL
UTILITIES LOCATED AND MARKED.
2.THE SITE IS SERVED BY MUNICIPAL WATER AND PRIVATE ONSITE SEPTIC
SYSTEM. CONTRACTOR SHALL COORDINATE WITH THE LOCAL WATER
DEPARTMENT FOR ACCESS TO WATER DURING CONSTRUCTION. USE OF A
BACKFLOW PREVENTER MAY BE REQUIRED. APPLICATION AND PAYMENT OF
ALL ASSOCIATED FEES IS THE RESPONSIBILITY OF THE CONTRACTOR.
3.CONTRACTOR SHALL COMPLY WITH OSHA TRENCHING AND SHORING
REQUIREMENTS FOR ALL UTILITY TRENCHING AND INSTALLATION.
4.WATER SERVICE INSTALLATION SHALL CONFORM TO MUNICIPAL AND/OR
LOCAL WATER DEPARTMENT STANDARDS.
5.THE CONTRACTOR SHALL COORDINATE WITH ALL UTILITY COMPANIES TO
DISCONNECT AND CAP ALL UTILITIES SERVING FEATURES TO BE
DEMOLISHED.
6.ALL NEW ELECTRIC AND COMMUNICATION LINES SHALL BE INSTALLED
UNDERGROUND IN CONFORMANCE WITH THE UTILITY COMPANY'S
SPECIFICATIONS.
7.ALL CONCRETE FOR ENCASEMENTS SHALL HAVE A MINIMUM 28 DAY
COMPRESSIVE STRENGTH OF 3,000 PSI.
8.ANY UTILITY FIELD ADJUSTMENTS SHALL BE APPROVED BY THE ENGINEER OF
RECORD AND COORDINATED WITH THE APPROPRIATE LOCAL UTILITY
COMPANY.
9.CONTRACTOR SHALL PROVIDE MARKING TAPE FOR ALL UNDERGROUND
UTILITIES.
10.ALL SEWER SERVICES SHALL BE PVC SDR-35 PIPE, WITH A MINIMUM SLOPE
OF 2% UNLESS OTHERWISE NOTED.
11.A MINIMUM OF 10 FEET CLEAR HORIZONTALLY SHALL BE MAINTAINED
BETWEEN WATER MAINS AND SANITARY SEWER MAINS AND/OR STORM
DRAINS. WHEN CONDITIONS PREVENT ACHIEVING A SEPARATION OF 10 FEET
LATERALLY, THE WATER MAIN SHALL BE LAID IN A SEPARATE TRENCH AND
THE VERTICAL SEPARATION BETWEEN THE UTILITIES SHALL BE AT LEAST 18
INCHES.
12.ALL WATER PIPE SHALL BE COPPER (TYPE K) UNLESS OTHERWISE NOTED.
EROSION CONTROL NOTES:
1.EROSION AND SEDIMENTATION CONTROLS SHALL BE INSTALLED PRIOR TO
CONSTRUCTION AND SHALL BE ADEQUATE TO PREVENT EROSION AND
SEDIMENT MIGRATION FROM THE SITE. ANY MODIFICATIONS TO EROSION
AND SEDIMENTATION CONTROLS PROPOSED IN THESE PLANS SHALL BE
INSTALLED IN ACCORDANCE WITH THE MASSACHUSETTS 2003 EROSION &
SEDIMENT CONTROL GUIDELINES FOR URBAN AND SUBURBAN AREAS.
2.EROSION AND SEDIMENT CONTROL PRACTICES MUST REMAIN IN PLACE
THROUGHOUT CONSTRUCTION UNTIL FINAL SITE STABILIZATION IS
COMPLETE AND VEGETATIVE COVER HAS BEEN ESTABLISHED.
3.DISTURBED OR EXPOSED SOIL AREAS SHOULD BE TEMPORARILY STABILIZED
USING SEEDING OR MULCH AS SOON AS POSSIBLE. NO PORTION OF THE SITE
SHOULD REMAIN UNSTABILIZED FOR MORE THAN SIX MONTHS OR LONGER
THAN NECESSARY TO COMPLETE THE ASSOCIATED WORK.
4.EROSION CONTROL DEVICES SHALL BE REGULARLY MONITORED,
MAINTAINED, AND REPLACED OR REPAIRED AS NEEDED THROUGHOUT
CONSTRUCTION. INSPECTIONS SHALL OCCUR WEEKLY AND IMMEDIATELY
BEFORE AND AFTER EACH RAIN EVENT
5.ALL DEWATERING OPERATIONS MUST DISCHARGE DIRECTLY INTO A
SEDIMENT FILTER AREA.
6.DURING CONSTRUCTION, AREAS OF EXPOSED SOIL THAT WILL NOT BE
COVERED AND GRADED TO FINISH GRADE SHALL BE STABILIZED AS SOON AS
PRACTICABLE BUT NOT MORE THAN 14 DAYS AFTER THE CONSTRUCTION
ACTIVITY. TEMPORARY EROSION CONTROL MEASURES SHALL INCLUDE
EROSION CONTROL MESH, NETTING OR MULCH AS DIRECTED BY THE
ENGINEER. IF MULCH IS USED, STRAW MULCH SHALL BE APPLIED AT THE
RATE OF 4 BALES PER 1,000 SQUARE FEET. APPLICATION AREA SHALL BE
COVERED WITH MULCH TO AVOID VISIBLE SOIL EXPOSURE. .
7.IF DISTURBED AREAS DO NOT RECEIVE FINAL SEEDING BY OCTOBER 1ST OF

FOR STORMWATER MANAGEMENT SYSTEM:
CONSTRUCTION PHASE (TEMPORARY):
1.THE BEST MANAGEMENT PRACTICES (BMPs) IDENTIFIED FOR THIS PROJECT
WILL BE OWNED AND MAINTAINED BY DESERT DE ORO FOODS, INC., WHO
WILL BE RESPONSIBLE FOR THEIR INSPECTION, OPERATION, AND
MAINTENANCE.
2.THE CONTRACTOR SHALL INSTALL AND MAINTAIN ALL DRAINAGE
INFRASTRUCTURE IN ACCORDANCE WITH THE SITE PLANS PREPARED BY
MCCLURE ENGINEERING COMPANY.
3.EROSION AND SEDIMENT CONTROL MEASURES SPECIFIED IN THE PLANS
MUST BE INSTALLED PRIOR TO ANY SITE DISTURBANCE. TO PREVENT
SEDIMENTS FROM MIGRATING OFFSITE, SEDIMENT BARRIERS SHALL BE
PLACED ALONG SLOPES THAT DRAIN TOWARD OPEN CHANNELS OR
ADJACENT PROPERTIES. THESE MEASURES MUST BE MAINTAINED
THROUGHOUT ALL PHASES OF CONSTRUCTION.
4.ALL DRAINAGE CONSTRUCTION WORK SHALL BE SUBJECT TO REVIEW AND
VERIFICATION BY THE TOWN OF YARMOUTH AND/OR AN AUTHORIZED AGENT
TO CONFIRM COMPLIANCE WITH THE APPROVED DESIGN.
5.THE REQUIRED SEQUENCE FOR CONSTRUCTING THE DRAINAGE SYSTEM IS
AS FOLLOWS:
A. CLEAR AND GRUB THE AREAS DESIGNATED FOR DETENTION SYSTEMS.
B. INSTALL DETENTION SYSTEM, CATCH BASINS, DRAINAGE PIPING,
STRUCTURES, AND MANHOLES.
6.EROSION CONTROL DEVICES SHALL BE CHECKED DAILY. IF SEDIMENT
ACCUMULATION IS OBSERVED IN CATCH BASINS OR OTHER STRUCTURES,
REMOVAL SHALL OCCUR WITHIN 24 HOURS.
7.ALL EXPOSED EARTH SURFACES SHALL BE STABILIZED IMMEDIATELY WITH
HAY MULCH.
8.INLET PROTECTION MEASURES, AS DESCRIBED IN THE APPROVED PLANS,
MUST BE INSTALLED IMMEDIATELY AFTER CATCH BASIN INSTALLATION AND
REMAIN IN PLACE UNTIL FINAL PAVING.
9.PRIOR TO THE CONSTRUCTION OF ANY IMPERVIOUS SURFACES, ALL
DRAINAGE PIPING AND STRUCTURES MUST BE INSTALLED AND INSPECTED
TO ENSURE PROPER FUNCTIONING. DURING CONSTRUCTION OF OTHER SITE
COMPONENTS, DRAINAGE SYSTEMS SHALL BE INSPECTED DAILY AND
CLEANED OR REPAIRED AS NEEDED UPON IDENTIFICATION OF SEDIMENT
BUILDUP OR DAMAGE.
10.FOLLOWING COMPLETION OF PAVING, PAVED SURFACES SHALL BE CLEANED
BY SWEEPING ON A MONTHLY BASIS.
11.WEEKLY SITE INSPECTIONS SHALL BEGIN WITH INITIAL SOIL DISTURBANCE
AND CONTINUE THROUGH FINAL STABILIZATION. FINAL STABILIZATION IS
DEFINED AS ACHIEVING 70% VEGETATIVE COVER IN ALL DISTURBED,
UNPAVED AREAS. INSPECTION LOGS SHALL BE MAINTAINED THROUGHOUT
THIS PERIOD.
POST CONSTRUCTION PHASE (PERMANENT)
THE OWNER SHALL BE RESPONSIBLE FOR CONTINUED MAINTENANCE OF ALL
