(BSC) is filing this Request for a Certificate of Compliance (RCOC) on behalf of Paul Buter of Jessie's Lane LLC (the applicant) for a project that was completed at 29 Parker's Neck Road, South Yarmouth, Massachusetts (the Site). The Order of Conditions (OOC SE83-2049) was issued on February 231d, 2016, and two OOC extensions have been issued. The first EOOC extended the permit until 2022 and the second EOOC extended the permit until 2025. The Massachusetts Leads act further extends the permits for two additional years. The approved project was to replace the existing single-family home with a new single-family home with a deck, porch, garage, driveway, utilities, grading, and new Title V septic system with retaining wall. The project has been completed in significant compliance with the approved project plans and the OOC. The only wetland resource area on Site is flood zone. Therefore, the house and garage are elevated for flood zone compliance and stairs were added to allow access. The only minor deviations from the approved plan are that (1) access stairs to the garage, the front door, and back deck have been added, as well as (2) two raised AC units on either side of the house. Included with this application are the RCOC checklist, Site photos, as -built plan, and a $75.00 check for the local filing fee. This application is being submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law.
