Ojala, P.E., P.L.S. Arne H. Ojala, P.E., P.L.S. Daniel E. Gonsalves, P.E., S.E. Craig J. Ferrari, E.1.T., S.E. Phellyp T. Tonial, E.I.T., S.E. In 1979, the Commission issued an Order of Conditions for the construction of a single-family dwelling and associated septic system. A site visit was made in March 2026. All work has been completed, and the house construction was found to be in substantial compliance with the approved Site Plan. There are, however, noted deviations from the approved special conditions. The driveway has a paved surface rather than gravel. Roof runoff is not directed towards drywells. And deck and patio areas have been added beyond the approved work limit line. It is noted that a deck of similar footprint was granted a special permit for encroaching into the rear setback in 2015 which decision is recorded in Deed Book 29393 Page 344. An as-built site plan and photos are enclosed for review. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
