Dear Building Commissioner,
I hereby formally withdraw my name,my Construction Supervisor License,and any and all association
from Building Permit No.BLDC-25-132 for the property located at 759 Route 28,Yarmouth,
Massachusetts,effective immediately.
I will no longer serve as the permit holder,contractor,or responsible party for this project.As of the date
of this letter,I disclaim any and all responsibility and liability for any ongoing or future construction
activities,inspections,code compliance,or work of any kind performed at the above-referenced
property.
Any continued use of my name,license,or credentials in connection with this project is unauthorized
and without my consent.
Please update your records accordingly to reflect my removal from this permit.No further inspections,
approvals,or correspondence related to this project should be directed to me.
Should any additional documentation be required to effectuate this withdrawal,please advise.
