I am writing to formally request removal of my name and license as the responsible permit
holder for the building permit listed.
Effective immediately, I am no longer associated with or responsible for the work being
performed at the above-referenced property. I respectfully request that your records be updated
to reflect my removal as the licensed contractor of record for this permit.
Please note that I will not be liable for any work performed on this project from the date of this
notice forward. It is my understanding that the property owner will need to secure a new licensed
contractor and ensure proper re-permitting or transfer of responsibility in accordance with local
regulations.
If you require any additional documentation or formal forms to process this request,please let
me know and I will promptly provide them.
