Ojala, P.E., P.L.S. Arne H. Ojala, P.E., P.L.S. Daniel E. Gonsalves, P.E., S.E. Craig J. Ferrari, E.I.T., S.E. Phellyp T. Tonial, E.I.T., S.E. In March 2021, the Commission issued an Order of Conditions for the construction of a bulkhead retaining wall, re-construction of existing stairways and pier, and a porch addition to the existing dwelling. This order was extended in 2024 and currently is set to expire 6/15/2027. A site visit was made in April 2026. The proposed work was found to be constructed in substantial compliance with the approved Site Plan. Minor deviations were noted including a new walkway along the side of the house and tree removal within the bank. A review of the Conservation file indicates that these matters have been resolved with administrative approvals in 2021, 2022, and 2025. An as-built site plan and photos are enclosed for review. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
