Notes:
S.W. Cole Engineering Construction Services Technician Robert Reuter arrived onsite at 6:30 AM for "Concrete Field Testing"
and for the placement location referenced above. Upon arrival, met with Eric Salo with CDM Smith who provided information
about today's job duties.
Concrete was delivered by "Cape Cod ready mix." and was 1-1/2” 4500 PSI Concrete. A total of (240) cubic yards of concrete
was placed.
A sample was collected from load 2,12,21 and the standardized testing was performed with; three sets of (5) cylinders were
cast from the load for laboratory compressive strength testing at 7, 14, and (2) 28 days. One cylinder was made for 'Hold'
purposes.

Observations and Results of the Inspection were continually communicated to Client Representative Eric Salo from "CDM
Smith ". Once the inspection was completed RR left the site.
