4. Install(1)400 amp 208/120V panel board in NEMA 4X enclosure
5. Wire lighting and Power for new proposed prefabricated toilet facility located at the
fields.

Good afternoon, Bill. I think it would be better to keep both projects on separate permits. That being said the field work,
as described on your attachment, I will need to know how many inspection visits that you are requesting. As for the
auditorium I will also need to know what you have in mind for the "fix" for the aisle lighting and how many inspections
that will require.
You also need to address the re-inspection fees that are due as of this date. (Four re-inspections @ $80.00 each for a
total of $320.00)
Let me know as socn as you can, and I will try to get back to you in a timely manner.

Good afternoon Mr. Elliott,
The general contractor was asking me about a permit for the auditorium at Dennis Yarmouth Middle School. If you can
let me know how much that fee will be, I will get it down to you tomorrow along with a permit application.
Additionally, when we met in February, you asked me to send a brief description of what we would be doing for the
additional field lighting work, (See attached description).
Does it make sense and is it convenient for you if we put both the auditorium and field lighting on the same permit?
Please let me know at your earliest convenience. I know the school administrators are hoping t o use the auditorium for
" end of school year" activities.

Good morning Mr. Elliott,
Don has informed me that you will be requiring additional funds for inspections going forward. Can you please call me at
your earliest convenience and let me know what we need to do to keep you on board for inspections as we are looking
to close the project out.
Additionally, I reached out to you on June and July last summer regarding a change order we were awarded to install
athletic field lighting and if you would require a new permit for that work. I don't believe I ever heard back from you. I
am attaching the permit application for the field lighting that I mailed to your office in July. If a separate permit is not
required, that's fine,just let me know.
