On March 12. 1026. an estimated 25-gallon release of waste oil i,ras discovered at the Yarntollth Waste

a tote containing rvaste oil that had been placed into the metal recvcling pile in the davs prior. u,as

palretnent. and a portion ofthe oil entered a downgradient catch basin. The afllcted catch basin r,vas punrped
and securcd overttight rvith a boom to address an observed sheen. and the paventcl.lt and rollolf contairrer
rvcre cleancd ttsing absorbents and degreaser. Roadway sedinrents within pavcnrent cracks in the relcase
arca \\crc assessed. and the largest of the cracks, that also ),ieldcd the highest fPH lleld scl'cening reading
was tat'sctcd litr additional assessnrent. After rernoving a section of- paverrcnt. a soil sartrplc collected
beneath the crack u'as submitted lor EPH laboratory analysis. Petrolcunr levels rvcre rnininral to non-
detectablc. u,e ll belorv the applicable MCP Method I risk standards. arrd are consido'ed to be approaching
backgrotrnd (non-detectable) levels. No soil. groundrvater. or surlacc u,ater rcnrediation u,as reqrrired- and
no onsoins source ol-contarnination remains. As a result. a level of No Signiticarrt Risl< rvith respect to
hutnan lrealth. public u,elfhre. and the environrlent has been achieved lbr this rcleasc. Furtlrenlore. no risk

On behalf of the Yarmouth Department of Public Works (Yarmouth DPW), I aurhorize Lisa lrwin, ofClean Harbors Environmental Services, lnc. (Clean Harbors) to sign Massachusens Department of
Environmental Protection (MassDEP) Bureau of Waste Site Cleanup transmittal forms, reporrs, bills of
lading and/or uniform hazardous waste manifests, waste profiles and waste disposal applications as Agent
for the Yarmouth DPW. for RTN 4-0053187.
This authorization is in accordance with Section 310 CMR 40.0009(2) of the Massachusens Contingency
Plan- I also authorize Clean Harbors to make electronic submittals of MassDEP documents. I understandthat Yarmouth DPW, remains fully liable under federal and slate laws and regulations regarding
Certifications of Person Undertaking Response Actions contained in the MassDEP tiansmittal forms as
the generator and responsible party, and that Clean Harbors would be signing solely for our convenience.
