On behalf of my clients, Tony and Jane Campagna, I am submitting 8 copies of additional materials as
required by the agent’s review of the project. Electronic copies of all documents will be emailed to the
conservation office. The following items are enclosed:
▪ Revised Notice of Intent Application (marked up in red ink; I can’t change an EDEP form online
▪ Revised Performance Standards Narrative to include the existing patio and deck work
▪ Check #2589 for $300 for the Town of Yarmouth By-Law fee (Category 1, patio and deck; after-the
fact $150 x 2 = $300)
▪ DRAFT revised plan “Plan Showing Proposed Bulkhead Reconstruction”, by Shorefront Consulting,
dated 7/23/25, revised 5/15/26 to show the NearMap aerial superimposed on the site.
Please include this information for your June 4, 2026 public hearing. If there are any questions or concerns
