Notes:
As requested by Rayssa (CDM Smith), S.W.COLE rep was onsite to observe concrete placement
and perform concrete sampling and testing at the locations referenced above.
The S.W.COLE Construction Services Technician arrived onsite at approximately 5:30 AM.
Concrete placement activities were observed during the time of sampling and testing.
Concrete Delivery No. 1 arrived onsite consisting of approximately 11 cubic yards of 4,500‑psi
concrete with 1‑1/2‑inch aggregate, which was placed via pump truck.
Three sets of concrete tests were performed with the following results:
Truck 62 – Load 1
Slump: 8.5 inches
Air Content: 4.5 percent
Concrete Temperature: 65°F
Truck 64 – Load 11
Slump: 8.5 inches
Air Content: 4.5 percent
Concrete Temperature: 65°F
Truck 58 – Load 23
Slump: 8.5 inches
Air Content: 4.7 percent
Concrete Temperature: 67°F

Three sets of compression test cylinders were cast for laboratory testing at 7, 14, and 28 days
(two cylinders per test age), along with one hold cylinder.
Test results and observations were verbally reported to the onsite client representative while
onsite. The S.W.COLE representative departed the site at approximately 11:45 AM after
completing the requested services.
