On behalf of my clients Ronald and Linda Ludvigsen, please accept this request for a Certificate of
Compliance for the above referenced project. The order was issued on 12/27/2018 and then recorded at the
Barnstable Registry on 1/22/2019 in Book 31794, Page 149. A site inspection was performed by Shorefront
Consulting on June 11, 2025. Pictures of the completed bulkhead and gravel buffer strip are included with this
request. Special condition #18 in the order requires a 4-foot buffer strip of gravel along the bulkhead.
The proposed bulkhead reconstruction work was completed in December of 2020. The bulkhead work was
performed in accordance with the plan. The floats were modified to be 5’x20’ per the plan. The landing was
reconfigured to approximately 7.3’x7.8’ where 6’x8’ was approved. The approved square footage was 48 sq.
ft. where the as-built square footage is approximately 57 sq. ft. The approved 10’x12’ pervious patio was not
installed and the gravel buffer was adjusted to maintain 4’ around the bulkhead and the shed as per the order.
There are no other deviations from the plan observed during the time of inspection. An As-built plan prepared
by Shorefront Consulting and stamped by Existing Grade is included with the Certificate of Compliance
request. Therefore, we certify that the work that was completed was performed in accordance with the order of
conditions and plan of record provided with the exception as noted.
The project has been completed in accordance with the Order of Conditions, and all Special Conditions contained
in SE-83-2182 issued on 12/27/2018. Attached please find Yarmouth form WPA-8A Request for Certificate of
Compliance, photos of the completed project, and a check for $75 made payable to the Town of Yarmouth.
Please include this request in your next public hearing. The original Certificate of Compliance should be mailed
to Shorefront Consulting for recording. If there are any questions or concerns regarding this request, please

Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
