Notes:
As requested by Rayssa (CDM Smith), an S.W.COLE rep was onsite to perform construction
observation, concrete sampling, and concrete testing at the locations referenced above.
The S.W.COLE Construction Services Technician arrived onsite at approximately 5:00 AM.
Concrete placement activities were observed during the time of sampling and testing.
Concrete Delivery No. 1 arrived onsite consisting of approximately 11 cubic yards of 4,500‑psi
concrete with 1‑1/2‑inch aggregate. Sika 75 admixture was added prior to placement. The
concrete was placed via pump truck.
Three sets of field tests were performed on sampled concrete with the following results:
Truck 79 – Load 1
Slump: 8 inches
Air Content: 4.2 percent
Concrete Temperature: 73°F
Truck 71 – Load 10
Slump: 9.5 inches
Air Content: 4.6 percent
Concrete Temperature: 73°F
Truck 63 – Load 18
Slump: 8 inches
Air Content: 4.2 percent
Concrete Temperature: 73°F

Three sets of compression test cylinders were cast for laboratory testing at 7, 14, and 28 days
(two cylinders per test age), along with one hold cylinder. Laboratory compressive strength
test results will be reported separately upon completion of testing.
Test results and observations were verbally reported to the onsite client representative while
onsite.
The S.W.COLE rep departed the site at approximately 3:15 PM after completing the requested
services.
