Responded from home lo reported structure tire in west Yarmouth. Ordered a working fire assignment
enroute based on reports trom the scene. Car 51 on location assumes command. Approximatety 30 x
20 1 1/2 hatf story home, no garage. Solar panets on back half of the roof.
E47 had taken a lra nsiliona t attack on the fire trom Side C F(oor two window. Completed 360 of the
structure, tire appears to be in floor 2 bedroom on B / C corner with extension into the atlic space.
Ctosed Side A and Side D doors on Ftoor 1 that had been left open to control ttow path.
Second due engine (42) to establish a water suppty to E47, take a second tine through side A and make
an interior attack.
Hyannis Engine on scene, had them tadder Side C floor 2 window, then conduct primary search of lhe
struclure to confirm alloccupanls had exited.
Engine 45 on scene assigned initial RlT. Tower 4'l crew assisled with initial evaluation of 2 occupants ot
the home untilarrivatot COMM 324.
Fire under controlin approximatety 30 min, extensive overhaut.
Various crews worked to overhautsecond ttoor, btown in insutation and contents of tire room att
removed through second Ftoor window.
Once overhauI comptete, worked wilh residents to remove some vatuables. Gas secured, Eversource
removed meter and disconnected servace drop at the pote.
Fire watch to remain on scene untitarrivatof Fire lnvesLigator in the morning.
Command terminated, units clear, scene left with L! Moriarty in E48.

FIRE INVEsTIGATION REPORT 8Y LT MATT BEARSE
NOTIFICATION AND RESPONSE: I was notitied by phone of the incident at 9 GittAve. I was unabte
to respond until 5am the next morning. I arrived to find Eng 48 on scene with a fire watch detait,
3 members oF the fire Dept. standing by monitoring tor hot spots. I met Lt. Moriarty on E-48 and
he advised me that at the last check about an hour ago, the buitdinq was ctear of smoke and no
hot spots were identified. E-48 cteared the scene.
BUILDING INFORMATION: The home is a single-famity residential home, sptit level cape styte
home. 34' X 26' with aLtached deck on side D. total square foolage '191 2 sq/ft. Buitt in 1977. 3
bedrooms wiLh partially finished basement.
FIRE SCENE INVESTIGATION: I began my investigation with proper PPE and began a systematic
approach working outside the home, then moving inside working my way from less burned to
most burned areas. I had a 5-9as meler with me and the air quality was ctear and safe. The room
oF origin is the bedroom on the side B/C corner. The room was fully overhauled by tire crews
prior to my arrivat. Att contenEs oF the room were moved outside. The interior sheet rock was
comptetety removed by tire crews.
INTERVIEWS: I spoke with the owner's Daughter Metissa Cook (508-728-9444) later that day on
the 26th. I met with her and her husband (Frank Cook) around noon time. Metissa stated to me
that she and her husband have been tiving in their RV on the property since Cotumbus Day to
assist her mom with upkeep of the house and hetp organize tinancial matters. They were aterted
by the dogs in the camper barking and immediatety tooked outside to see flames coming out the
bedroom window on side B. Her husband grabbed a fire extinguisher from the RV and entered
through the side C door and weot upstairs to the bedroom and was pushed back by high heat
and heavy smoke, the ftames were concentraled to the nightstand and wallaround the Side B
window, as wetl as the bed. The extinguisher did nothing to stow down the Fire. Metissa calted 9-
1-1, the owner Shirtey Argiros, was stitt in the house atong wilh Metissa's brother Christian.
Metissa was totd by lhe fire dispatcher to get everyone ouE of the house, which they attdid
evacuate saFely to side A. A[[ parties were evatuated at the scene by EMS and refused treatment
or transport.
I spoke to lhe owner of the home, Shirtey and her son Christian (508-237-4600) on the foltowing
Monday 12130/24. They met me al the house atong with Melissa and her husband. Shirley did not
recatt much of the events on the oight ot the tir€ and was mitdly confused about the
circumstances. Shirtey did not know what coutd have started the fire. Shirtey betieves that maybe
a strip ptug was used for the night tabte tight and window fan. She did not know if the fan was
on, she woutd use lhe tan when she smoked in her bedroom. She occasionatty used atcohol and
did have "Tito's" vodka in her room, atong with some wine on the night of the fire. Shirtey woutd
smoke both selt-rotted and store-bought cigaretles. Shirtey was awakened in her room by the
noise of the tire and was abte to get out prior to being injured. Shirtey does not have a cetl phone
and could not give me a reason why she didn't catt 9-1-1. Christian stated to me tha! he was
aterted by his mom of the Fire and stated the smoke alarms were sounding. Christian attempted
to extinguish the Fire with water bolttes at the same time as Frank was using the extinguisher.
FIRE CAUSE AND CONCLUSSION: The Origin of the tire is the side B/C bedroom along the side B
wal/window area. The bed frame and maltress were burned comptetely and the side tables
showed burn patterns atl the way to the Ftoor, as they were both on either side oF the bed.
Potentiat ignition sources are caretess disposaI of smoking materiats, electricaI arching trom
eilher a pinched cord or strip ptug and polenlial for the window fan motor faiture. I was unabte
to determine the exact cause of the tire or whal item was tirst ignited. The fire is undetermined
in nature and presumed accidentaI at the lime of this report.

E-42 dispatched to a buitding fire. on arrival, E-42 laid a supply
from a hydrant !o E-47. E-42 then stretched an atlack line from
E-47 into the front door on the Atpha side of the buitding for
fire attack and overhaul on the second ttoor.
E45 was Fourth unit to arrive on location. Met with command
and moved to over-watch side-c. crew performed multipte
lasks beginning wirh medicaI evatuation of home's occupants;
and assisting with overhaut.
Catted for a struclure fire with peopte inside. 47 on [ocation
with fire showing trom the Bravo side 2nd ftoor window and
smoke from the rear Charlie side- 47 passed command. 47
deptoyed a l 3/4" tine to the Bravo side exlerior knocking the
Fire down and keeping it from spreading to the rest of the
house. An occupant informed us that everyone was out of the
house- 51 on location and assumed command. Fire spread lo
the attic. Solar panets were on the rear roof. 42 on localion,
secured a water suppty, and brought a second line to the
second ttoor.45 and 41 on scene.52 on scene.47 crew
knocked down as much as possibte from the exterior and the
went interior to assist 42 crew. MutuaI aid crews oo scene. Fire
was extinguished and overhau( compteted. A fire watch arrived
and atl companies returned.
Engine 48 responded as Fire watch to retieve duty crew.
Monitored buitding overnight For any possibte hot spots and
retained custody of home untit Fire inspector arrivat.
