(BSC) is pleased to submit this Request for Determination of Applicability (RDA) Application on behalf of Joesph Lapiore (the Applicant), for a project located at 20 Perch Pond Way, (the Site) in Yarmouth Port. The proposed project involves the construction of an addition to a single-family dwelling. This addition will be constructed over an existing lawn and landscaped area within the outer 100-foot Buffer Zone to the Inland Bank of Perch Pond and NHESP Priority Habitat 337. This application is being submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law and associated Regulations. The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Contact the Conservation Office if you need assistance. Submitting an Application: Does the property have any outstanding/expired Orders of Conditions? If so, please file a Request for Certificate of Compliance for each outstanding/expired Order, as the Commission may not issue a DOA until any outstanding/expired Orders have been closed and recorded.
