Notes:
As requested by Rayssa (CDM Smith), an S.W.COLE representative was onsite to perform
construction observation services, including concrete sampling and testing, during concrete
placement operations at the project site.
The S.W.COLE Construction Services Technician arrived onsite at approximately 6:45 AM.
Concrete placement activities were observed during the time the S.W.COLE representative was
onsite performing the requested sampling and testing services.
Concrete Delivery No. 1 arrived onsite consisting of approximately 10 cubic yards of 4,500‑psi
concrete with 1‑1/2‑inch aggregate. Sika 75 admixture was added prior to placement. The
concrete was placed using a pump truck.
One set of field tests was performed on sampled concrete with the following results:
Truck 75 – Load 1
Slump: 7 inches
Air Content: 4 percent
Concrete Temperature: 72°F
One set of compression test cylinders was cast for laboratory testing at 7, 14, and 28 days (two
cylinders per test age), along with one hold cylinder. Laboratory compressive strength test
results will be reported separately upon completion of testing.
Test results and field observations were verbally reported to Rayssa (CDM Smith) while onsite.
The S.W.COLE representative departed the site at approximately 7:45 AM after completing the
