Notes:
As requested by Reisa, S.W.COLE rep. was onsite to perform construction observation services,
including concrete sampling and testing, during placement operations at the project site.
The S.W.COLE rep. arrived onsite at approximately 7:10 AM and departed at approximately
1:30 PM after completing the requested observation, sampling, and testing services.
Concrete placement operations were ongoing during the time the S.W.COLE rep. was onsite
performing services.
Concrete Delivery No. 1
Concrete Delivery No. 1 arrived onsite consisting of approximately 11 cubic yards of 4,500-psi
concrete with 1-1/2-inch aggregate. Sika 75 admixture was reported to have been added prior
to placement. The concrete was placed via pump truck.
Three sets of field tests were performed on sampled concrete with the following results:
Truck 60 – Load 6
Slump: 6 inches
Air Content: 4.5 percent
Concrete Temperature: 75°F
Truck 65 – Load 13
Slump: 8 inches
Air Content: 3 percent
Concrete Temperature: 70°F

Truck 54 – Load 20
Slump: 8 inches
Air Content: 4.5 percent
Concrete Temperature: 70°F
Three sets of compression test cylinders were cast for laboratory testing at 7, 14, and 28 days
(three cylinders per test age), along with one hold cylinder.
Laboratory compressive strength test results will be reported separately upon completion of
testing.
Concrete Delivery No. 2
Concrete Delivery No. 2 arrived onsite consisting of approximately 11 cubic yards of 4,500-psi
concrete with 3/4-inch aggregate. Sika 75 admixture was reported to have been added prior
to placement. The concrete was placed via pump truck.
One set of field tests was performed on sampled concrete with the following results:

Slump: 9.5 inches
Air Content: 4.5 percent
Concrete Temperature: 75°F

(three cylinders per test age), along with one hold cylinder.
Laboratory compressive strength test results will be reported separately upon completion of
testing.
Test results and observations were verbally reported to Reisa, CDM Smith, while onsite.
