On location in Tower 41, positioned Tower on Side A of buitding. As I exited my apparalus numerous people were tetting me that a man in a wheetchair was trapped on the second ftoor. I entered the buitding wilh a standpipe hose kit and made my way to the second ftoor. The man in Ehe wheel chair was nowhere to be seen. I lhen proceeded to lhe third floor where the tire apartmenE was. Engine 45 Lt. met me on the third floor and we forced the door and found heavy smoke with one sprinkter head activation. There was a smallfire stittvisibte in the cabinets over the stove. Engine 45 Lt. did a search which was negative. I returned !o side A as I was command and more people had arrived on scene. I shut down the sprinkter main valve as I exited the buitding. Engine 45 used Tower 41 to provide a water source For standpipe handtine. Passed Command to Depufy Sawyei. E45 driverlump seat assigned wafer suppty and stand by for assignment . E45 LT mee! Cap!. Huck at apartment with water flow and forced door and encountered heavy smoke and low hea! with sprinkler flow Performed primary search and rescued two felines from apartment. Fire above stove observed during search held to area by sprinkle . Secondary was perFormed with no victims locaEed . E45 jump and driver met at Fire ttoor for extinguishment and vent . Depuly Chief Sawyer: Upon arrival I mel with Captain Huck, was briefed on current situation and then I assumed command. Yarmoulh Engine 45 had secured a water supply, Tower 41 was positioned and deployed a handtine via standpipe kit and water suppty provided by the bucket of Tower 41. Sprinkler activation held tire in check untiI handline was charged and ready for use. Sprinkler system waler supply was shul off at the main vatve by Caplain Huck. Fire was extinguished and Engine 45 and Tower 41 crews did extensive overhaul. Engine 42 and Dennis Engine 1 12 crews were ordered to do a primary search of all Ftoors. Lack of keys prevented a 100% search. One FF was transported aFter a syncopalepisode. Two residents were transported for minor medicals. Secondary search was compteled much later due to key issues. Buitding Commissioner and Etectrical Inspector arrived and dectared the buitding inhabitabte. Red Cross arrived to assist tenants. Monday Red Cross opened a shelter, shetter was not used at attand was ctosed Wednesday at 2. I arrived later in the incident on Amb. 55 to stand by and assist as needed. After the fire was extinguished, I was assigned to Fire lnvestiga[ion and requesled my work vehicle !o the scene tor needed equipment. White ouLside I spoke with a person who stated her apartment was the apartment that had the fire. Mary Derick D.O.B. 8-26- 91 (774-552-8833) Mary stated to me that hersett and boyfriend (George Boudreau DOB 6-4-96 508-585-2057) were not home at the time of the fire, they were aterted by a neighbor by phone. They were shopping at a locatstore. Mary Stated to me that they had lunch prior !o leaving the apartment and used a Air-Fryer to warm two pieces of pizza. She remembers turning oFf the air- fryer and pushing it back on the stove. she stated she did not unptug the apptiance. The oven was not used at all that day. She has had the air-fryer tor approx. 2 years and has not had any issues with it. Both Mary and George did not notice anylhing unusuaI today with any power surges or etectricaI problems. They warmed the pizza 15 minutes prior to leaving aparEment, and were catted about the Fire, 15-20 minutes aFter leavinq the apartment. Their main concern was for their 2 cats which were rescued and given to them by the Firefighters. I asked if any other devices or apptiances were ptugged in the etectricaI outlet with the air-tryer lo which both Mary and George said nothing etse was ptugged in. I began my investigation by taking pictures of the apartment 2307 and found the area of origin to be in the kitchen in the area of the stove. I coutd not determine if the apptiance was plugged in or not due to the heavy Fire damage to the electrica I outtet. The knobs were burned off the oven on the side of the oven where the appliance (the menlioned air-fryer)was burned and melted. Therefor i coutd not determine if the oven knobs were in the on or ott position. The microwave was ptugged in above in the cabineE. I requested lhe Electrical lnsPector lo the scene (Ken Ettiot) and he and I removed the etectrical box and outteE From the wallto have a ctoser inspeclion. The etectricat panetwas in the bedroom and att of the breakers were in the off position, which I was totd [ater was done by a Firetighter during fire suppression. i spoke to the tenants after my preliminary investigation, I asked it any candtes were used, Mary said no. They did not store any discarded cigarettes' in the area of origin. The area on counter next to stove is generatty open to store the air fryer on lhe counter. Mary said, maybe an oven MIT was on the counler. I am unabte to determine the heat source that contributed to lhe initiat ignition of the tire At the time of this report, I believe !he tire is accidentat in nature, with potentiaI heat sources oF the oven burner being mistakenty turned on or an air fryer tha! may have been mistakenly turned on and left unattended.
