Dear Yarmouth Conservation Commissioners,
BSC Group, Inc. (BSC) is filing this Request for a Certificate of Compliance (RCOC)on behalf of the Joseph Lapriore
(the applicant) for the project at 20 Perch Pond Way in Yarmouth Port, Massachusetts (the Site). The Order of
Conditions(OOC SE83-1390)was issued on February 20'h, 1997 and referenced a plan titled "Site&Sewage Plan of
Lot 6, Perch Pond Way, Yarmouth Port, MA" with a final revision date of 2/20/1997. The approved project was to
construct a single-family home.The OOC does not require the submission of an As-Built plan with the RCOC and does
not require any mitigation plantings.
BSC completed an existing conditions survey of the property on April 8'h, 2026, and can confirm that the single-family
home was constructed in compliance with the OOC. The single-family home is located slightly landward from the
location where it was proposed;i.e.further from the wetland resource area.The only other deviations from the approved
plan are that there is a small paver patio behind the house, an outdoor rinse station attached to the back of the house,
and a small block retaining wall along the backside of the driveway. These three features are all located near the limit
of the 100' buffer zone and since the house itself was constructed landward of the approved location, the total buffer
zone impacts on Site are similar to what was approved. See the attached plan overlaying the proposed plan with the
existing conditions plan to see that the house is located further landward than was proposed as well as the other minor
deviations. All general and special conditions listed in the OOC have been followed including Special Condition #7
which requires that gutters and downspouts are directed into drywells.
This application is being submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of
Yarmouth Wetlands Protection By-law. If you have any questions or need additional information, please contact our
