As requested by Wright-Pierce, S.W.COLE rep. Andrew Carter was onsite at approximately 5:20
AM to observe concrete placement operations and perform field testing at the location
referenced above. Upon arrival, the S.W.COLE rep. met with Rayssa, representing CDM Smith,
to discuss the planned concrete placement activities for the day.
Concrete was supplied by Cape Cod Ready Mix Inc. and was reported to be a 3/4-inch nominal
maximum aggregate, 4,500-psi design mix with SIKA Control 75 admixture.
A total of approximately 116 cubic yards of concrete was placed during the observation
period. Concrete sampling and field testing were performed in general accordance with
applicable ASTM standards.
Samples were collected from the first and tenth loads delivered to the site. The following
results were obtained:
First Poured Load – Truck No. 78
Slump: 7.5 inches
Air Content: 3.4 percent
Concrete Temperature: 84°F
Tenth Poured Load – Truck No. 60
Slump: 8.25 inches
Air Content: 3.0 percent
Concrete Temperature: 82°F
Concrete placement was observed to be performed using a pump truck. Internal consolidation
was observed using a worm-type internal vibrator. Concrete spreaders were observed being

used to distribute the material, and hand tools, including trowels and a bull float, were
observed for leveling and finishing operations.
A total of five (5) concrete cylinders were cast from the first load and tenth load (10 total
cylinders) for laboratory compressive strength testing, consisting of one (1) cylinder at 6 days,
one (1) cylinder at 14 days, and two (2) cylinders at 28 days. One (1) additional cylinder was
designated for hold purposes.
Field test results and observations were verbally communicated onsite to Rayssa,
representative of CDM Smith, at the time of testing.
