Hello Maureen,
It was a pleasure speaking with you today. As discussed, your home has an Innovative Altemative (I/A) septic
system. This system requires regular maintenance w'ith a certified provider and your JET system is under
warrarity. We are the provider that will be taking care of the warranty period for your JET system. Your system
is set up for routine semi-annual operation and maintenance service in February and August. Your Jet system is
covered for 2 years O&M (operations & maintenance) service visits at no cost to you. The Effluent field
sampling and reporting - $108.00 per activity ($432.00 for fwo years) has been prepaid by the installer. There is
an annual Bamstable County I/A Septic Management Database access fee ($60.00) & a Clearwater Recovery
administrative fee ($60.00), both are mandatory for an annual cost $120.00.
I have enclosed literature, your prepaid invoice and our standard O&M (operation & maintenance) contract for
your review and signature. Please sign and return the contract to our of/ice. Once we receive your signed
contract, \ ,e will countersign the contract. send you and the board of health a fu1ly executed copy. We would
like to offer a courtesy, no charge visir, with you for a walkthrough to meet and answer any questions you may

Please keep in mind that the Deparfment of Environmental Protection requires homeowners of
innovative/alternative (IA) systems to maintain an active Operations and Maintenance contract with a
liceased service provider, for the life of the system ' (For .more informttion, please see
