SLAT SIGN APPLICATION

Application is hereby made to install a Slat Sign on an existing rack in accordance with the Town of Yarmouth Zoning
Bylaw Section 303.4.2.1.,.the Selectmen's Slat/Ladder Sign Policy 90-11 (as.amended),and MGL Section 8,Chapter 85.
Permit is issued by the Building Department pursuant to the below listed. Only Slat Signs that have been issued valid
permits and have been placed on the prescribed racks in locations approved by the Yarmouth Department of Public Works
will be allowed:
The applicant agrees to abide by the terms of these laws and regulations. Slat Sign permits may be revoked at any time
for any infraction of these laws and regulations.
Please Note:
1) Applicant Business must be located within 2 miles of rack location,
2) A Slat Sign Application must b.' -ubmitted for each Slat Sign to be placed on rack.
3) The Building Department shall be notified in writing within ten(10)business days of any change to the above
information.
