Yarmouth — Certificate of Compliance Up2rade Septic, Reconstruct Deck Closed In Porch To the Conservation Commission, Attached please find Form 8A and a check in the amount of $150. The Order of Conditions issued in 1992 was for an upgrading the septic system, reconstructing an existing deck and closing in an existing porch. The COC certification is limited to the project outlined in the Order. From a site inspection made on June 25, 2026 and review Yarmouth Dept. of Health and Conservation records the septic system approved by the Order was installed following issuance of the Order of Conditions. A 1999 Title 5 Septic System Inspection Report on file indicates the system was installed consistent with the plan referenced in the Order. The septic system work was a condition of proposed work on a porch. From a review of the files, it is not clear where the porch is located however the condition to work on it appears to be met by the system installation. Based on my research, knowledge and belief, and inspection of the site, I hereby certify that the project was carried out in substantial compliance with the Order of Conditions and the approved plan.
