In December 2021, the Commission issued an Order of Conditions for the above-referenced site, for the construction of an addition to an existing single-family dwelling. A site visit was made and a survey of the existing conditions relative to the Order was performed. All work to the house has been completed but the required mitigation was never planted. An expanded mitigation area is proposed with a new Notice oflntent related to a proposed pool. An as-built site plan and photos are enclosed for review. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions aside from the above mentioned deviation.
